We are honored that Mural Health has been named a finalist for the 2025 SCOPE Summit Participant Engagement Award (PEA), for our work to make clinical trials more accessible and diverse through the Harley Jacobsen Clinical Trial Act. In order to promote a more diverse trial population and drug development process, we need to make it possible for all Americans to participate. The Harley Jacobsen Act is a key step in that mission. Currently, the 110M Americans on a social welfare program are prevented from participating in trials, because those payments are very often mislabeled as taxable income, which threatens to disqualify low-income Americans from receiving government benefits like Medicaid and SNAP. The Harley Jacobsen Act seeks to change this by excluding all payments - to all participants - in all clinical trials from being treated as taxable income. Endometriosis Australia welcomes the announcement by the Hon Mark Butler to support individuals with complex gynaecological conditions such as endometriosis and polycystic ovary syndrome. The data from the Australian Institute of Health and Welfare (AIHW) suggests that most care in complex gynaecological conditions, such as endometriosis, occurs in the private sector. As such, individuals affected by such health conditions already face extensive out of pocket costs. Right to Die with Dignity: The recent draft guidelines by the Ministry of Health and Family Welfare on withdrawing life support for terminally ill patients aim to operationalize the Supreme Court's rulings from 2018 and 2023 on the right to die with dignity. These guidelines provide a structured framework for state governments and hospitals to ensure ethical and lawful end-of-life care. Key mechanisms include establishing Primary and Secondary Medical Boards within hospitals to assess whether continuing medical treatment benefits the terminally ill. A secondary board, nominated by district officials, will review and confirm decisions made by primary boards. The concept of withdrawing or withholding life-sustaining treatment involves stopping interventions such as ventilators or feeding tubes when they no longer provide meaningful recovery. This aligns with the Supreme Court’s acknowledgment of the right to life and dignity under Article 21 of the Constitution, extending it to decisions about one’s end-of-life care. "Living wills," legally recognized under the 2023 ruling, empower individuals to predefine their preferences for end-of-life treatment. These must be documented and signed in the presence of witnesses and notarized for legal validity. They help doctors and families navigate difficult decisions, ensuring patients' wishes are honored. However, these processes are not about giving up but respecting the patient’s dignity, avoiding unnecessary suffering, and fostering open communication between families and medical professionals. The Profit Motive in Healthcare: A Barrier to Patient Welfare In the healthcare sector, particularly within certain gazetted health institutions, there is an unsettling trend where profit motives overshadow patient welfare. The influence of grants from the Ministry of Health (MOH) can often lead to decisions that prioritize financial sustainability over the holistic well-being of patients. When treatment protocols and medication choices are driven by financial considerations rather than the best interests of patients, the quality of care can suffer. This focus on ward survival and organizational efficiency may compromise the individualized care that patients need for genuine recovery and support. It is essential that we advocate for a healthcare system that prioritizes patient welfare above all else. Transparency in decision-making and a commitment to patient-centered care should be at the forefront of healthcare policies. I was pleased to join Lisa Dean and Melanie Perks at Invox's National CHSP Summit last week to discuss 'Engaging staff & boards on the change journey'. Our key discussion points as they relate to boards included: 1. Sector Support and Development has produced a Board Briefing Toolkit; a great resource to ensure the information your board is receiving covers key changes. Ensure your board understands they have a strategic leadership responsibility. This includes setting the strategic direction of the organisation, within the context of the big picture, which includes the CHSP/S@H reforms. 3. Boards, senior teams and other leaders must understand the current reforms are not occurring in isolation. They are part of a broader trend by government that has been occurring for decades – the delivery of human services, think child care, disability, employment, mental health, allied and primary health etc, via managed marketplaces. The commercialisation and corporatisation of human services. 4. In this big picture of the continuing move to a customer-driven competitive marketplace for human services delivery the current CHSP/S@H and wider aged care reforms are another key driver that will shape the industry, and likely move home care in all its forms into the Preferred Provider phase (refer model below for a pictorial view of the move to a customer-drive competitive marketplace, noting some of the provider numbers in home care and residential care are a touch out of date; the numbers are less now!). 5. CHSP organisations and their boards were perfectly adapted for the previous government funded welfare paradigm and with the new Act resetting the rules they must transform their board to ensure it can support and lead the transformation of the organisation.
