💡 Is Advanced Care Planning (ACP) defined and implemented effectively? 📊 Recent literature shows that patients’ future end-of-life (EOL) goals may be transient, and thus the effectiveness of ACP in facilitating goal-concordant care has come into question. 🔍For ACP to remain relevant, re-definition and re-branding of ACP is required: · Changing ACP to “Advance Care Preparation” can help clarify that its purpose is not only EOL planning. · The mission should be trifold, to Educate, Share and Prepare patients for the future. The focus should be on iterative conversations with patients about their health, illness and present values, rather than documentation of definitive goals of care for the future. · ACP also needs to be better integrated into routine care, with simplification of documentation and responsibility being shouldered by all stakeholders, to normalise ACP and ensure that conversations are not only conducted during an emergency. The IHI just defined the role of the Health Equity Officer (HEO) and they drop this 💥 right out of the gate in the guide (paraphrasing): 💥 HEALTH EQUITY OFFICER ≠ DEI OFFICER AND 💥 making a call to move it OUT of institutional HR to advance equity as an organizational priority. Many respondents suggest the HEO report to the CEO. ----------------------- TLDR: They call out the importance of empowering the HEO with authority and a level of decision-making and link Health Equity closely with Quality, which I would especially expect coming from IHI. This will probably grow as more CMS and other accrediting bodies release "health equity" measures. 3 "Conditions for Success": - Equity Is a Strategic Priority - Equity Leaders Hold Positions of Power - Resources Are Invested in Improving Equity ----------------------- MY TAKE: - Data analytics play an understated role. They mention stratification which is a great start but just scratches the surface. - I see it being a challenge to have authority and a level of decision-making over other Departments in an org, and I'm never convinced Quality measures should be a sole driver of organization's strategic change, but their ability to encourage (require) things to move/change are clear. - Health equity isn't just about low income minority groups, but pervades all aspects of health and social systems. It impacts all patients in some way, not just the ones we refer to as "marginalized" (though they certainly are disproportionally impacted in many areas). - That is why "Health equity" as a concept is fundamentally just good healthcare, aligning with the Triple Aim of improving patient experience, health of populations, and reducing costs. I don't see it as political fodder; it's about recognizing that the same way we have been doing things clearly is not working for everyone and particularly not for those who continue to fall through the cracks. Navigating the Intricacies of Prior Authorizations in Healthcare Services In my two decades of dealing with healthcare revenue, and having first-hand experiences of a quadruple bypass and a hip replacement, I can confidently lay down some facts about Prior Authorization (PA). A common misconception about PA is that it's notoriously knotty - it is in fact quite straightforward if you follow the correct procedures. When you're providing healthcare services, you must meet the patient's health plan's criteria for those services to be "authorized.” The main concern with PA is often the lack of documented reasoning or justification for the procedure. To get it right, ensure that the clinical documentation, test results and care plan paint an accurate picture of why the procedure is necessary. Should a PA request be denied, it's usually due to insufficient clinical documentation or prerequisites. Health plans often need to see that other less invasive or expensive methods have been tried and failed before approving a more serious procedure. This isn't just a cost-saving measure for the healthcare provider and insurance company, it's also critical in ensuring the best patient care. For instance, before approving surgery for back pain, health plan usually prefers that the patient undergoes physical therapy first. Not only is this financially beneficial, but it is also less distressful for the patient. It's therefore crucial that while applying for the PA, one should list the patient's accurate diagnosis codes, the planned procedure, and corresponding CPT codes. Turned down payment requests, despite an "authorized" procedure, often result from inconsistencies between what was "authorized" and what was actually performed. It could be data entry errors or coding discrepancies. Therefore, cross-checking and confirming details while filing a PA is crucial. Here are a few suggestions to streamline the PA process; 1. Engage an external service to regularly audit your records. 2. Hire a certified coder. 3. Let your certified coder or clinical staff handle PA submission. 4. Today the Centers for Medicare & Medicaid Services (CMS) published the request for applications (RFA) for the ACO Primary Care Flex (PC Flex) Model, which will test #primarycare hybrid payments within MSSP beginning January 1, 2025. To apply, ACOs must submit an MSSP application (as an initial, re-entering, or renewing/early renewing applicant) and a supplemental PC Flex application questionnaire. The MSSP application is open now through June 17 at 12:00pm (noon) ET. The PC Flex supplemental application must be submitted by August 1, 2024. CMS will host a model applications webinar on June 6 at 2-3:30pm ET. Key model details: > The model will run January 1, 2025 – December 31, 2029. > Eligible participants are low revenue ACOs using prospective assignment that begin a new MSSP agreement starting Jan. 1, 2025; ACOs cannot simultaneously participate in Advance Investment Payments (AIP). > Participation is at the ACO level and all ACO participants and their primary care providers will be required to participate; fee reductions will be applied at the TIN/NPI level. > In addition to the MSSP application and PC Flex questionnaire, applicants must submit a spend plan, roster of non-physician practitioner data, and assurance of ability to repay; applicants may withdraw the PC Flex application at any time prior to signing the Participation Agreement (December 6 – 12, 2024), and may withdraw the PC Flex application without impacting the MSSP application. > Participants will be required to submit a health equity plan, collect and report health equity data, and secure a repayment mechanism (separate from MSSP repayment mechanism for 2-sided tracks). > ACOs will distribute the Prospective Primary Care Payment (PPCP) to their participant primary care providers, FQHCs and RHCs. When comprehensive and accurate, clinical documentation tells the true patient story, serving as a primary source of health data. This ensures a patient’s care team – now and into the future – has all the information they need to deliver the best possible care, for the best health outcomes. 🌟 🪙 Not to mention the benefits accurate documentation brings to the healthcare bottom line.
