We’re excited to announce our value-based care collaboration with Aetna, a CVS Health Company, is expanding into Texas. Now eligible Aetna Medicare Advantage members across the state will be able to benefit from our comprehensive, home-based care for chronic and end-stage kidney disease. This is especially important in a state like Texas, which has the fourth highest rate of kidney disease in the country and the largest percentage of dialysis patients in the entire U.S. Since Monogram Health started in 2019, we’ve steadily increased the number of patients we reach, and this announcement marks another milestone. We now cover over 200,000 patients across 36 states and look forward to improving outcomes for more Americans in the months and years to come. #ValueBasedCare #AccessToCare #ChronicDisease
